  1. Latest color passport-size photograph (white background, front-faced, without headgear or sunglasses).
  2. Self-attested copies of degree certificates and mark sheets issued by recognized authorities.
  3. Self-attested copy of matriculation certificate for proof of age.
  4. Self-attested copy of APST certificate / PRC / Aadhaar Card (for general candidates).
  5. Self-attested copy of appointment order to current post.
  6. Treasury challan copy of application fee payment.
